There is no guarantee the ticket will get waived; the 'Fail to Surrender Insurance Card' ticket is not for driving without insurance, it's for not producing an insurance card promptly to a police officer when requested.

The #1 problem with Google Maps, Earth & Streetview images is that the pictures were taken a long time ago. The intersection you received the offence notice may look different.

Also submitted the city road map report with the roads categories and speed limits and it was totally ignored by the Justice(what a joke). The road was listed as category B and speed limit 80 outside urban area-the Justice accepted the prosecutions statement that in the city speed limit is 50km/h just by saying so.

1) ignore the ticket
2) option 2 - early resolution
3) option 3 - trial date
All three paths have the same conclusion if you don't appear; a justice of peace will have to review the face of the ticket and quash it if there is a fatal error.

Most police officers follow the rules of the road whether they're in a marked cruiser or a civilian car. They set the example for everyone else on the road. IMO there would be no jurisdiction issue there, it's all City of Toronto for at least a 2km radius. Lets just say that 50 km/hr sign was not there, the Highway Traffic Act would default the speed limit to 50 km/hr anyway.

Normally if there are fatal errors on the ticket, the clerk at the ticket office will cancel them. When the building owner went through MLEO training, they were instructed to initial corrections.
If it does go to trial, keep in mind that the Provincial Offences Act allows the courts to amend the ticket.

You should go back, and re-submit it; tell them you were there the previous day... they will most likely go and get your 11b application and add your affidavits of service.
It wouldn't hurt to add the offence number and the date/time/location of your trial date. Just make sure that you have your copy stamped 'received'.

Maybe you can describe exactly where and what the sign was?
Disobey Sign is very generic charge, all the enforceable signs erected on the road are protected by it. ie: Speed Limit, HOV, Do Not Park, etc. Depending on what the sign read, and where you did it... there may be a lot of other 0-point charges to choose from.

The other driver is 100% at-fault for the collision if he tried changing lanes. You should inform your insurance company of the accident, and describe exactly how it happened.
Police don't necessarily assign fault for collisions, they're already pre-determined through 'Fault Determination Rules' under the Insurance Act.
