What happens if you don't pay a speeding ticket from out of town? Does it only affect your licence plate renewal? What are the consequences, if any?

In simple terms:
You will be found guilty, a notice of conviction will be sent, then a notice if suspension will be issued and sent by mail.

Just to clarify, what do you mean by out of town? And if you're asking about another Province are you talking about photo radar or a regular ticket where you were charged as the driver?

I got the ticket in another township 3 hours away from where I live. Same province. And it was by radar I assume. I was driving in the opposite direction of the cop when she turned her lights on.
