I tend not to follow lane etiquette at all in the 401 collectors - it's just too difficult. If I need to get over to Express, I'm in the left hand following the flow, one of the few times I might get up to 140 km/h. I was under the impression that you do not have to pull over right there and then if you cannot identify the police vehicle.

For example: you are driving alone in the middle of the night on an intercounty highway with no lights. Next thing you know a car in front lights up like a cherry XMas tree. No other indication that the car is a police car ...

I wouldn't want to give my license to an ununiformed LEO. Off duty means that. There are rules against that.

Signs are present on overhead bridges, though don't think they are bilingual in the GTA. I have only seen the "This lane" sign a few times - most of them are overhead.

But "turn to the left" is confusing when you are referring to lane changes. :P

What are the provisions for painted lane markings for designated lanes? Many times I see no erected signs anywhere, just painted arrows on the ground itself. Valid?

There is a police report, which is why I was a bit surprised when the broker said it was under comprehensive. I was pretty much expecting that, but still surprised. Ultimately, I don't think the trucking company will even get a slap on the wrist, as they will just ignore the bill, ignore the collection agency, and then the insurance company will ...

Disagree. First I've heard of changing lanes to allow a left-turning vehicle in. You have the right of way, just stick with it. Doing people "favours" tends to just lead to confusion and misinterpretation. You will have better luck going after your insurance company to cover it under DCPD, especially since you have the plate number and a police report. Quote them everything you can find on commercial (or even non-commercial) vehicles needing to secure their load while on a highway.
