Is there anywhere I can get the overleaf of an MVA report to see what the number/boxes the officer checked on the out of the written/diagram area of his report. Thank you

Not knowing where you live or the accident happened: Try a collision reporting centre or go to local police station, they would likely let you look at one...

I highly doubt it. If the codes were meant to be read by the drivers then the key would be provided.

There are no big secrets with the codes, it is not sensitive material at all: Weather Conditions, Traffic Control, things like that...I can't see why they wouldn't

They have a copy of the chart on the MTO website here: http://www.mto.gov.on.ca/graphics/engli ... t-back.gif
The reports are now in an electronic format but I believe the codes remain unchanged.
