Received my disclosure notes; need help deciphering
Can someone help me decipher the notes I received? I especially need to know what happened in that bottom section about the Laser Atlanta. Based on the notes, it looks like it was only tested once in that day? can someone comment on that?
Here's a copy of the disclosure. I believe that it contains no officer information in there. at least not from what i can make out.

I think its stating that its Laser Atlanta serial number 22347 I'm guessing T/D is tested device M/S is myself W/O is ? at 0610 Station, 0847 at Setup and 1215 at takedown. Looks like 3 times noted. Most of the other shortforms I can make out but a few are unknown.
You may want to ask for a deciphered interpretation of the notes.
