Defence In Writing - Help

I filed an appeal in a city other than Toronto, where I got a ticket.


I got a ticket for speeding, then they set the trial date. I wasn't able to attend so I requested it be adjourned by fax. They changed the date but I was never informed of the change of date. I got a notice in the mail that I was convicted.


I appealed. They accepted to hear the case. But now I need to present in writing the reasons for requesting a new trial.


Is there a sample or form somewhere? Likely lots of people are in this situation. Is this what is called a factum? I have no clue how to start or what to say. Should I just say what I wrote above, with better grammar, of course?
