Need help in teaching my grandmother her faults

My grandmother has been driving for years and is horrible. I witnessed her attempting to parallel park downtown and took notes on what she did.
I am looking for help in identifying ALL her faults with valid violation names and would appreciate any help. I am hoping that [presenting her with a list of ALL the faults may make her realize that some additional training is needed.
